APPLETON, WI (WTAQ) — The City of Appleton has announced the launch of the Appleton Community Concerns Survey.
This initiative, spearheaded by the Department of Public Health in collaboration with local stakeholders, aims to gather insights into the needs and concerns of those who live, work, and play within the city.
The survey is a component of the broader Community Health Improvement Plan, scheduled for development later this year. Its objective is to identify key areas for health and wellness improvements by directly hearing from community members themselves.
Residents are encouraged to share their views on a variety of topics, including healthcare, education, safety, environmental concerns, and more. The information collected will help establish the priorities and actions that will be created in the Community Health Improvement Plan.
Dr. Chuck Sepers, Health Officer at the Department of Public Health in the City of Appleton, emphasizes the importance of community participation: “This survey is a cornerstone of our efforts to ensure that every resident’s voice is heard. Their input is invaluable as we strive to make Appleton a healthier, safer, and more vibrant place to live. We strongly encourage everyone to take part and help shape the future of our community.”
The deadline to submit the survey is April 30.

Results from the survey and updates on the Community Health Improvement Plan will be shared with the public as they become available.
